A New Way to Manage System Forms Launching Oct. 14

Information Services is launching a new Forms Management System, creating a centralized location for managing and accessing approved system forms in OnBase.

Why this change?

Forms are used throughout Cook Children’s every day but managing multiple can make it difficult to ensure team members are accessing the most current, approved version. The new Forms Management System will provide a one-stop shop for forms, making it easier to:

  • Find and print current, approved forms
  • Request a new form or changes to an existing form
  • Identify form ownership and maintain appropriate oversight
  • Manage form revisions and version history
  • Support standardized review and approval processes
  • Receive notifications when forms are approaching their required review date
Supporting compliance and keeping forms current

Forms must be routinely reviewed to ensure they continue to meet operational, regulatory, legal, and organizational requirements. Cook Children’s requires forms to be reviewed at least every two years in alignment with The Joint Commission (TJC) requirements. The new system will help automate this process by notifying form owners when a review is approaching, providing greater visibility and accountability throughout the form lifecycle.

What does this mean to you?

Once the new system is live on Oct. 14 team members will have a centralized resource for finding approved forms and initiating forms-related requests. This will help reduce duplicate and outdated forms, improve consistency across the system, and provide greater confidence that the form you are using is the current, approved version.

Where can I get support for a form after Oct. 14?

After Go-Live the link for the Request Form and OnBase Library will be located in the CCHCS-Medical Records Forms Committee home page. There are Knowledge Base Articles (KBA) in ServiceNow. To access: type in OnBase or Forms Mgt. in the search and the KBA’s will be listed. If you have OnBase issues submit a request through ServiceNow.
