Prosper Playground Now Open for Patients
We unveiled the new playground at Cook Children’s Medical Center - Prosper at a special event last week. Before letting the kids loose, our staff had the job of finding out how fun the new playground is. APP Fellowship Applications Open
Are you interested in becoming an Advanced Practice Provider (APP)? Apply to the new APP Fellowship Program! This fellowship is designed to support the clinical and professional growth of highly motivated nurse practitioners and physician assistants. The program emphasizes hands-on learning through skills practice, simulation, and real-world patient care experiences to help foster confidence and clinical proficiency. New Medical Record Forms Committee CookNet Site Available
Bookmark the newly launched Medical Record Forms Committee CookNet site. This site will serve as the central location for all Forms Committee–related resources, including: - Current and archived medical record forms
- Committee policies, procedures, and guidelines
- Meeting schedules
- Form submission, review, and approval documents
All committee members and relevant staff are encouraged to use this site to access up-to-date information and collaborate on Medical Record Forms Committee initiatives. Important Updates & Reminders - Outdated Forms Review: The Forms Committee will be reviewing forms last updated prior to January 2024. Identified form owners will be contacted to coordinate required updates and ensure continued compliance.
- Form Owner Responsibility: If you are a form owner and notice that a form you are using appears outdated or inaccurate, please contact us at MedicalRecordFormsCommittee@cookchildrens.org so we can assist with coordinating the review and update process.
- Paper Forms: Anyone who has a stash of paper forms should destroy them immediately and obtain the most current version from the appropriate CookNet page, if needed.
- Electronic Forms: Electronic forms available within Epic should be used whenever possible.
- Approval Requirement: All new forms and any changes to existing forms must be reviewed and approved by the Forms Committee before they can be used.
- Posting Forms: PDFs of forms should not be posted directly on CookNet pages. If a form needs to be referenced on a page other than the “Consents and Other Medical Record Forms” page, it must be done using a link back to the official version.
- Ongoing Review: All medical record forms are required to be reviewed at least every two years. To learn more about our processes and procedures, please check out policy, QM347.
Please update your bookmarks and begin using the Forms Committee CookNet site for all Medical Record Forms Committee activities moving forward and to access the most up-to-date versions of Medical Record Forms.
