P&D Performance Dimensions & Rating Training for Leaders 

Are you interested in learning more about the performance dimensions used in Performance & Development, and how to accurately evaluate your team members? The P&D team has created a class that enhances understanding of the performance dimensions, offers a process to consider ratings, and provides best practices about how to complete year-end performance evaluations.

  • 5 p.m., July 30
  • Noon, Aug. 4
  • 2 p.m., Aug. 5
  • 8 a.m., Aug. 6
  • 10 a.m., Aug. 8

Peer Feedback Has Begun!

If you were selected to provide feedback to your peers, please complete it by July 27, 2025.

Your feedback is for developmental purposes only. It does not impact performance evaluation scores.

Tips:

  • Share your appreciation for their work or behavior and use positive, uplifting language to encourage continued excellence.
  • Provide constructive feedback by describing ongoing actions that can be taken by your peer to improve performance. 
  • Your feedback should provide fact-based, actionable performance insights.

FY25 Year-End Performance Evaluations are Just Around the Corner! 

Self-Evaluations start July 27

The year-end performance evaluation process kicked off on June 8, beginning with Peer Feedback, which was an opportunity for employees to provide insights to those with whom they worked most closely during the performance year.

As a reminder, Self-Evaluations will take place July 27 – August 10. The self-evaluation is an opportunity for employees to conduct an honest reflection of the contributions and impacts they have made during the FY25 performance year.

Employees are strongly encouraged to complete their self-evaluation to document their achievements and opportunities for growth. Self-evaluations are required for Magnet nurses.

Key Dates:  

  • July 27 – Aug 10: Employees Complete Optional Self-Evaluation (required for Magnet nurses)
  • Aug 10 – Sep 13: Managers Complete Performance Evaluation
  • Sep 13 – Sep 28: Managers/Employees Complete Year-End Discussion
  • Sep 28 – Oct 5:  Employees Acknowledge Year-End Discussion

The full FY25 Year-End Performance Timeline is always available on the Performance & Development site on CookNet, along with helpful tools and resources for both managers and employees.

Annual ULearn courses are due Aug. 1, 2025

Employees are required to complete the assigned Annual Competency modules in ULearn training as part of their year-end performance evaluation,

The deadline for completion is Aug. 1, 2025

Failure to complete the Annual ULearn courses will result in disciplinary action:

  • Failure to complete by deadline – Written Warning
  • Failure to complete within next 30 days – Final Written Warning
  • Failure to complete within next 14 days – Suspension
  • Referenced Policy HR 190 Mandatory Training, Education and Competency Validation

Optional Leader Feedback for Directors and below began July 13, 2025

If you were selected, this is your opportunity to provide feedback to your leader on the effectiveness of their coaching, leadership and teaming behaviors.

Tips:

Provide specific behaviors, leadership, coaching, collaboration and guidance that your leader exhibited that helped you grow and/or navigate your work successfully.

Also provide ways that your leader could cultivate a healthier more inclusive environment. Remember to keep it fact based and work related.

Leader feedback is due Aug. 3, 2025

Flu Vaccines 2025To-Do Friday Facts

To help protect our patient families, fellow employees and the community, all employees are required to receive the annual influenza vaccine by Oct. 17.

To start the process, the electronic consent forms become available on Aug. 4 on the Enterprise Health portal. Additionally, onsite Flu Vaccine Fairs will kick off Sept. 16
