TB Questionnaire and Respiratory Mask Fit Requirements
Requirement reminders for Performance & Development: Managers, as you complete Performance & Development (P&D) evaluations, please verify that employees are compliant with both the annual Tuberculosis (TB) Questionnaire requirement by asking them to complete the form inEnterprise Health and any required RespiratoryMask Fit Testing obligations.
TB Questionnaires: Employees who completed the TB Questionnaire during the August 2025 campaign remain current in theEnterprise Healthportal until the day after the annual due date of Sept. 14, 2026. Employees who have completed the TB Questionnaire during the current campaign will remain current until the day after Sept. 14, 2027.
RespiratoryMask Fit Testing: Employees who are currently late on their required RespiratoryMask Fit testing have been granted a final extension and must complete the requirement by Dec. 11, 2026. Please schedule your RespiratoryMask Fit test with Occupational Health as soon as possible. Appointments are filling up fast.

Reminder about Parking: 8th and Terrell Parking Garage is Closed
The 8th and Terrell Parking Garage is closed completely for parking. The sidewalk along Terrell Avenue between 8th and 7th avenues is closed to pedestrians.
If you previously parked in this garage, please check for an email sent from Parking Services to see your new parking assignment. You may NOT park in other lots or garages, per policy MC 252.
