COVID-19 Vaccine Appointments Available for Cook Children’s Patients Ages 5-11

You will need a MyCookChildren's Patient Portal to schedule

Throughout the pandemic, we have kept the safety of you and your family a top priority. With that in mind, we want to provide your children 5 to 11 years old the first opportunity to receive the Pfizer-BioNTech’s COVID-19 vaccine.

Only children in this age group are eligible to receive this version of the vaccine because it is one-third of the dose of the adult vaccine. Children will receive two shots at least three weeks apart.

Cook Children’s has received enough doses to vaccinate more than 3,000 children.

If you have a child in this age group, you can schedule an appointment at one of our community vaccine events. You will need a MyCookChildren’s Patient Portal to schedule your appointment. If you do not have a MyCookChildren’s Portal, please use this link to request one.

Cook Children's Physician Network (CCPN) is organizing a total of 12 centralized vaccination dates. Those events will be spread over four locations and offered one weekday (3 p.m. to 7 p.m.) and one weekend (8 a.m. to 2 p.m.) per location.

  • Nov. 10: Southwest Fort Worth, 6210, John Ryan Dr., Suite 104, Fort Worth, TX - 6132
  • Nov. 11: Renaissance
  • Nov. 12: Southlake
  • Nov. 13: North Carrollton
  • Nov. 14: Southwest Fort Worth
  • Nov. 20: Renaissance
  • Nov. 21: North Carrollton
  • Nov. 21: Southwest Fort Worth
  • Nov. 28: Southlake 

When you schedule your first dose, you will get the date and time for your second automatically. Your child's second dose will be 21 days after the first.
