Flu Vaccines 2026: Onsite Flu Fairs Begin Sept. 15

Flu vaccines are the same this year: Sanofi Pasteur’s Fluzone, Flublok and High Dose vaccines. They don’t contain MRNA.

All employees are required to complete the annual influenza vaccination process and receive their flu vaccine by Oct. 23, 2026.

Beginning Sept. 1, please visit the Enterprise Health Portal to complete your electronic influenza consent form.

All Cook Children's staff must complete the electronic consent form by Sept. 15, regardless of whether they receive their vaccine through Cook Children's or an outside provider.

Some departments will administer vaccines within their areas. Employees may also attend one of the Flu Vaccine Fairs listed below or receive their flu vaccine through an outside provider and submit documentation through the portal.

  • Sept. 29: 7 a.m. to 3 p.m., CCMC Atrium

  • Sept. 30: 8 a.m. to 5 p.m., 750 Building
  • Sept. 30: 7 a.m. to 3 p.m., 901 Atrium
  • Oct. 6: Noon to 3 p.m., ROB 1102-1103
  • Oct. 7: 7 a.m. to 3 p.m., Family Support Meeting Center

Receiving Your Flu Vaccine From an Outside Provider

If you receive your flu vaccine from a non-Cook Children's provider, such as a physician's office or pharmacy, please upload proof of vaccination through the My Medical tab in the Enterprise Health Employee Portal.

Please do not send documentation to the Occupational Health email account.

Documentation must include:

  • Employee name
  • Date of birth
  • Date of vaccination
  • Provider information
  • Vaccine lot number
  • Manufacturer
  • Which arm received the vaccine

Please note that sales receipts and "waiting for vaccination by pharmacy" forms cannot be accepted. Documentation must verify that the vaccine was administered.

Exemption Requests

Employees requesting a medical or religious exemption must submit an exemption request through the Enterprise Health Employee Portal.

Important deadlines and steps:

  • Exemption requests must be submitted by Oct. 16, 2026.
  • Select either a medical or religious exemption request in the portal.
  • Occupational Health will provide the appropriate forms.
  • Completed forms must be uploaded to the portal for review by the Accommodations Committee.
  • Medical exemption requests require physician documentation, so employees are encouraged to begin the process early.

The Oct. 16 deadline allows the Accommodations Committee sufficient time to review requests and make final determinations before the vaccination deadline.

Flu Vaccine Pickup for Department Events

Nursing departments across all companies may request vaccine from Occupational Health and administer vaccinations within their departments.

Flu vaccine availability is anticipated around Sept. 15. Please contact Kasee Williams and Jamie Kayser to coordinate pickup dates.

Location and date information will be communicated by Prosper Administration.

All Other Companies

Annual Flu Compliance

2026 Compliance Deadline: Oct. 23, 2026

Cook Children's Health Care staff are expected to meet annual influenza vaccination requirements. Occupational Health establishes and publishes compliance deadlines each year.

Employees who do not complete the vaccination requirement by the established deadline will be subject to the corrective action process outlined in Policy HR800:

  • Failure to complete by the deadline: Written Warning
  • Failure to complete within the next 14 days: Final Written Warning
  • Failure to complete within the following 14 days: Suspension

Annual Mask Fit Testing

Cook Children's Health Care System is committed to maintaining a safe and healthy environment for employees, patients and visitors.

Because we care for patients who may present with highly contagious communicable diseases, patient- and family-facing employees are required to complete an annual N95 respirator fit test. As demonstrated during the global pandemic, these situations can arise quickly, and it is important that employees remain prepared and protected.
