Employees are Invited to Participate in the Disability Self-Identification Survey

As an Equal Opportunity/Affirmative Action Employer, Cook Children’s Health Care System is required by law to: recruit, hire, promote and retain individuals with disabilities and to treat qualified individuals without discrimination on the basis of their physical or mental disability.

On an annual basis, Cook Children’s prepares an Affirmative Action Plan (AAP). The AAP contains statistical data regarding our workforce demographics. To assist in measuring this statistical data, we invite you to complete the three (3) question survey and self-identify as either, 1) a person having a disability; 2) a person not having a disability; or 3) I do not wish to answer.

The survey will be available on Dec. 16, 2024, for 15 days.  Upon completion of the survey, please select “Submit”; then a message will appear on the screen stating, “You have submitted." If you have any questions, please contact Mekka Livingston (ext. 54132).

Your participation in the survey through Workday is voluntary. All survey responses will be kept confidential and maintained in separate files in Human Resources.

An individual with a disability is defined as having a physical or mental impairment that substantially limits one or more major life activities or has a history or record of such an impairment (such as cancer that is in remission) or is perceived by others as having such an impairment (such as a person who has scars from a severe burn.

Major life activities are the kind of activities that you do every day, including our body’s own internal processes. There are many major life activities in addition to the examples listed here. Some examples include but not limited to:

  • Actions like eating, sleeping, speaking and breathing
  • Movements like walking, standing, lifting and bending
  • Cognitive functions like thinking and concentrating
  • Sensory functions like seeing and hearing
  • Tasks like working, reading, learning, and communicating
  • The operation of major bodily functions like circulation, reproduction, and individual organs

The following list of Physical or Mental impairments is from the Americans with Disability Act Amendment Act (2008):

A disability does not indicate that you do or do not require an accommodation to perform the essential functions of your position.  However, if you believe you are in need of an accommodation, please contact our Occupational Health department (ext.53837)

If at any time your disability status should change, you may voluntarily self-identify your disability status in Workday by going to your - Employee Profile, Action Tab, Personal Data & Change Self-Identification of Disability.
