Kindness, Respect and Generosity Take a Front Seat in Newly, Renovated Hochberger Auditorium

As a Cook Children’s employee, you are the heart and soul of our organization. Your co-workers, fellow staff members, retired staff, teammates and friends are important to you, and that makes them important to us. That’s why we are pleased to offer a new opportunity exclusively for Cook Children’s employees. You can honor the people who create moments of magic for you, your patients and your team by purchasing a Hochberger Auditorium chair plaque in their name. Located on the back of the chair, these personalized, engraved plaques are limited and sold on a first-come, first-served basis. Click here to view an example of the chair plaque. 

Plaques can be reserved by individuals or groups with a $1,500 donation to our Children’s Fund, which supports the most immediate needs of our patients and families across the health care system. If you wish to reserve a plaque, please complete this form.

Select a payment option

If you’d like to split the $1,500 donation between members of your team, please select the split payment option. This option allows for transactions that require dividing the $1,500 expense among two or more parties. The same primary contact must be listed on all corresponding split payments. The primary contact will have the final say in plaque details. Additionally, the sum of all split payment amounts for one plaque must be equal to or greater than $1,500.

The split payment option does not qualify for payment plans. If you split payments, please ensure the “recognition type” and “name” are the same on each form that’s completed for a group. The plaque must be paid in full before it is produced.
