New Notice of Privacy Practices: Nov. 1, 2026

We have made some changes to our Notice of Privacy Practices. The new one goes into effect on November 1, 2026. This is especially important for anyone who registers patients to be aware of.

The Notice of Privacy Practices is the four-page document that is posted in our lobbies, on our website (Notice of Privacy Practices), and on CookNet (Notice of Privacy Practices).

What you need to know:

  1. This version will replace all postings within Cook Children’s provider entities.
  2. This version will replace the current paper version in all hospital departments, all CCPN clinics, and at Cook Children’s Home Health and the Child Study Center.
  3. This version will replace all online versions.

(This change does not affect Cook Children’s Health Plan.)

Please throw away any paper versions of old Notices of Privacy Practices and make sure that, beginning Nov. 1, 2026, you are using the new one in situations where the notice is being provided to a patient.

We need to provide a copy of the Notice to new patients and at the annual renewal, or if an existing patient requests a copy prior to their renewal. When the Notice is provided to a patient, they also need to sign the Acknowledgment of Privacy Practices/Financial Disclosure form. However, please note that the Notice of Privacy Practices is different from the Acknowledgment of Privacy Practices. We will be changing the Acknowledgment, as well, and will let you know when the new one has been approved.

To better assist this transition, please remove any old paper versions of the Notice of Privacy Practices within your department or clinic. The only approved Notice of Privacy Practices will say, “Effective date: Nov. 1, 2026” on the last page. Any previous versions will need to be placed in the shred bin.

Please be on the lookout for the new version to be sent to you by your designated leader. If you have any questions, you can send those to Privacy.Officer@cookchildrens.org.
