Peak Performer: Rachel Salazar

Rachel has been with Home Health less than a year and from day one, she has been actively engaged and wanted to learn. Rachel is resourceful in obtaining information she needs to fulfill her duties and provides care to her department and incoming patient referrals. Rachel is constantly getting pulled in many different ways, but due to her organizational skills she is able to stay on top of the most fragile orders and minor details.

Rachel rounds on her department, which has helped reduce the amount of orders pending within the department for processing. Rachel is the first person to jump in and help with patient orders/care when staff call out to maintain patient care and prevent delays with discharges. Rachel is actively engaged with case management, as well as managers at other Home Health locations to streamline patient deliveries from each warehouse.
