Please help us recognize a deserving social worker for Cook Children’s Social Worker of the Year Award. Social workers serve in various roles across Cook Children’s, including CARE Team, Case Management, Center for Community Health, Clinical Therapy, Family Therapy, Genetics, Health Plan, Home Health, Intake, and more.

In celebration of Social Work Month, we are looking for a social worker who has gone above and beyond this past year to provide outstanding patient care. Nominees will demonstrate excellence in meeting Cook Children’s experience standards to connect, collaborate, and elevate, and uphold our core values (kindness, collaboration, imagination, generosity, respect, and safety).
