Registration is Open for Employee Craft Fair

Registration is now open for our fall Employee Craft Fair! 

Please submit your application and booth fee to Jana Jackson, Child Life Office Manager, located in the Medical Center- Fort Worth across from Gift Shop.  You can email the form in but the fee must be paid within one week or your space will be forfeited. 

Please send registration through interoffice mail if you are paying with a check. If paying with cash, please deliver in person. 

The number of booths is limited to space availability; these will be assigned based on who first submitted their application and other accompanying material. Those applications arriving after the spaces are filled will be placed on a waiting list.  Limit one booth per employee.
